Purpose:
We would like to learn more about your experiences with [Course/intervention X]. A team of instructors [in the Faculty/Department of X] are developing resources/making changes for students related to [topic]. As this is a new area for all of us, your feedback is very valuable to help us determine what is and is not working and make changes for future offerings.
Details of the survey:
You will be asked to complete a [XX minute] survey. Your participation in the survey is anonymous [amend if student IDs are being collected in the same survey], voluntary and will not impact your grade; you may stop responding to the survey at any time. Your instructor will not see the data until final grades have been submitted.
Incentive:
If using a grade incentive: At the end of the survey, you may choose to input your student ID to receive a 0.5% bonus mark on your [class participation mark, a particular assignment, etc.]. Please note that your student ID will be deleted before data analysis takes place. At the end of term, [team member/neutral third party] will provide your instructor with a list of participant IDs, without sharing any linked data.
If using a gift card incentive: At the end of the survey, you will be directed to a separate survey to input your name and email address to enter a prize draw for a $25 gift card. This will ensure your anonymity. [Consider gift cards to the UBC bookstore, local coffeeshops, and grocery stores.]
Note: If using a grade incentive, ensure that students who do not complete the survey or want their data included in the analysis have an alternative way of being eligible for the incentive. For example, provide a 5-10 minute reflection prompt linked in the survey that students can complete as an alternative.
Analysis:
Please note that your responses to this survey will be [de-identified/anonymized] and will not be traced back to you. We will not report any individual results; we may share [de-identified/anonymized] quotes and comments and will rephrase them to preserve anonymity where necessary. Your instructor will only have access to the raw, anonymous [if using gift card incentive] or [de-identified/anonymized] [if using grade incentive] data after all course grades have been submitted.
Data privacy and dissemination:
Data will be securely stored on encrypted computers and shared between the project team members using Sharepoint/OneDrive, a secure storage tool. Aggregate data and/or [de-identified/anonymized] quotes may also be shared publicly, published in academic journals, and/or presented at scholarly conferences.
Use of AI [If relevant]:
The project team may use AI-assisted tools [Tool name and link to tool privacy information] for analysis, including data cleaning and preparation, thematic analysis and visualization. All identifying and demographic information will be removed before processing. Human review and oversight will be integrated throughout to ensure accuracy, quality, and integrity. [Tool name] runs on local computers, and inputted data will not be used for training [using the data for training is not allowed, as per UBC BREB guidelines].
